It is strongly suggested that you download the latest version of Acrobat Reader. The forms for Briefs are provided in Word format. The Word template may be downloaded to your computer and completed by you.

Please note, App.R. 9(B)(3) and Loc.R. 9(B)(1) require a transcript order form to be filed with a docket statement if transcripts of proceedings will be ordered to be prepared for purposes of the appeal. Parties may use the forms found below under All Appeals.
